WHAT A SOCIAL WORK CASE MANAGER- TRANSITIONAL HOUSING-RAPID RE-HOUSING DOES:

  • Conduct client-screening interviews and assess service needs via phone and face to face interview
  • Assessment and case planning on each participant
  • Teach Independent Living Skills and provide support to participants in relational wellness, gaining employment and school opportunities among other areas
  • Provide crises intervention, situational and group counseling to residents and phone clients
  • Provide information and referrals to other community resources and services
  • Communicate effectively with Department Head, Program Director, co-workers, participants, families, and other service providers
  • Coordinate case planning with residents, clients and collaborating agencies
  • Complete and manage all assigned reports and timelines accurately and appropriately
  • Maintain organized records and case files for all participants
  • Follow up on discharged/aftercare clients
  • Develop and maintain community outreach regarding TAY Services programs and THYD Department, including volunteers and local businesses
  • Keep well-written; concise case notes to include all pertinent information for each participant using County and State mandated tracking systems
  • Provide a secondary on-call option during specified times
  • Ensure residents maintain safe and adequate living environment
  • Transport participants in YMCA vehicle to and from activities and appointments as needed
  • Operate YMCA vehicles in safe manner.
  • Work independently and as a team member to create innovative ways to meet contract goals and provide a high level of service that is engaging and meets the needs of the participants
  • Participate in and attend all required staff meetings, trainings, YMCA staff development events and appropriate agency-wide committees
  • Ability to work effectively with others in alignment with the YMCA 4 Core Values
  • Models the 4 Core Values in all aspects of position responsibilities

WHAT WE LOOK FOR IN A SOCIAL WORK CASE MANAGER- TRANSITIONAL HOUSING-RAPID RE-HOUSING:

  • Bachelor’s degree in social science, psychology or a related field is required
  • Must be 21 years of age or older
  • Minimum two years' experience working with young adults (ages 16-24) required
  • Familiar with the operations transitional housing and rapid rehousing
  • Ability to work in a very diverse setting with people of all backgrounds, culture and orientation
  • Experience with Independent Living Skills issues preferred
  • Excellent communication skills
  • Ability to provide excellent customer services and maintain a positive demeanor
  • Ability to build positive rapport and communicate effectively with participants
  • Ability to maintain clear and professional boundaries with participants
  • Must be self-aware, flexible, adaptable and teachable
  • Demonstrate initiative, resourcefulness and problem solving skills
  • Must be able to work independently and as part of a team
  • Basic computer knowledge and skills
  • Strong organization skills with the ability to work in an environment of rapidly changing deadlines
  • Must have reliable transportation to travel throughout San Diego County
  • Valid CA Driver's License
  • Good driving record
  • Current CDL H6 print-out (must be dated within 30 days of application.)
  • Pass pre-employment drug testing
  • Pass vehicle proficiency exam
  • Criminal record clearance by being fingerprinted prior to reporting to work and signing a statement regarding criminal convictions
  • A physical exam and negative TB test results
  • Must be an adult role model with the ability to effective set limits and function in crisis-oriented and high stress situations
  • Visual and auditory ability to observe critical incidents and the physical ability to act swiftly in an emergency situation
  • Ability to adequately observe participants' activities, enforce safety regulations and apply appropriate policies and procedures
  • Must possess the ability to lead and interact in group activities and perform related physical skills
  • Current CPR/AED and First Aid, both Adult and Infant/Child, certification required and must be from one of the following certifying organizations: American Red Cross, American Heart Association, or American Safety and Health Institute.
