|Position Summary||TheCenter for Development and Disability (CDD), established in 1990, is NewMexico's University Center for Excellence in Developmental Disabilities Education,Research and Service. These centers, authorized by the DevelopmentalDisabilities Assistance and Bill of Rights Act, build the capacities of statesand communities to respond to the needs of individuals with developmentaldisabilities and their families.|
The UNM/CDD Family Community Partnerships Division is seekingto hire 2 individuals to provide education and care coordination toparticipants with the Supports Waiver. The individuals hired for this positionwill help participants navigate through the new Medicaid Waiver processes. They will provide assistance and guidancewith eligibility, certification, pre-planning, service and support plandevelopment, budget development and budget implementation, and revisions aspart of their assigned caseload.
The individuals hired for this position will be responsiblefor maintaining weekly COVID well checks, monthly phone contact withparticipants and once quarantine restrictions are lifted, will be conductingquarterly home visits to see participants in their home and in the communitythat are on their assigned caseload. They will be responsible for keeping track of participants annualeligibility dates, and accurately documenting all contacts with participants, andproviders.
The individuals hired will be responsible for providingeducation and training to participants served including participation in thedevelopment of training aids and guides. The successful candidates will beworking with a dynamic team of professionals to meet the needs of participantsthroughout New Mexico.
These positions require statewide travel.
|Conditions of Employment|
Bachelor's degree; at least 3 years of experience directly related to the duties and responsibilities specified.
Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
