What You’ll Do

The Social Worker is responsible for patient advocacy, emergent counseling and education needs for patients and families, and effective discharge planning.

The successful candidate will be able to:?

  • Act as liaison between the patient and medical personnel
  • Facilitate long term health and well-being through counseling, education, and brokering of needed community services and resources
  • Develop and implement appropriate treatment plans for each patient
  • Serve as an advocate for patients and their families in regard to decisions affecting plan of care
  • Act as a resource for hospice and counseling services
  • Provide appropriate end of life planning and care
  • Ensure timely discharge planning

What You’ll Bring

  • Master’s of Social Work degree required
  • Previous hospital experience preferred?
  • Must have a passion for excellence?
  • Must demonstrate excellent written and verbal communication skills
  • Professionalism displayed through sound judgement, initiative, and flexibility?
  • Good interpersonal, communication, team-work, and customer service skills?
  • Strong organizational, problem-solving and follow-through skills?
  • Ability to multi-task constantly, react to changes quickly/productively, and tolerate interruptions?
  • Ability to perform work when no directions are given, and give directions to others?
  • Remain flexible, organized, professional and able to analyze, make sound decisions under stressful situations? ?
  • Demonstrate maturity and accountability?

Position Requirements:
This position has minimum requirement of an MSW from a program accredited by the Council on Social Work Education (CSWE). An active member of the National Association of Social Workers is also desired.
