Characteristics of Position:
All employees are expected to effectively support the agency'smission to provide consumer-centered care to those with substance disorders orwith co-occurring substance and psychiatric disorders. Employees areexpected to engage consumers in continuing care and treatment by usinginterventions that are diagnosis-specific, and communicate empathically withclients regarding addiction, mental illness, medications and recovery. Promotesclient's self-determination, personal responsibility and empowerment inherentin self-directed recovery; facilitates and teaches clients role recoveryelements. This is a full time position,based out of the Hillsborough Juvenile Assessment Center (JAC), located inTampa, Florida.

? Communications - Communicate withsupervisor and established chain of command regarding daily operations and/orincidents; document all required information in a clear, legible, timely,concise and accurate manner.

? LPN or a Bachelor's degree in a health or human services field
? All ACTS, Inc. employees must be able and willing to work whenexpected to accommodate program and/or agency needs, participate in theagency's Quality Improvement Program and attend all assigned/requiredtrainings, practice on-the-job safety at all times, and demonstrate the abilityto communicate and work with others and be respectful of peers, supervisors,clients, guests, etc. If required by contract or administrative rule, mustfully participate in NAPPI (crisis intervention) training and able to applysame to work setting.
Preferred Attributes: Experience working in education, counseling, or wellness coaching in amedical or behavioral health setting
Physical/Safety/Health Considerations: Standard officeenvironment and meeting rooms; work in community-based settings includingclient homes, primary care clinics, schools, etc.
Quality of Work Expected: Level commensurate withsatisfactory Health Coach performance
Quantity of Work Expected: Commensurate with level necessary tofully meet obligations
Scope of Responsibilities: Conduct psychosocial andhealth assessments; provide education and information in individual or groupsettings to identified clients; promote consumer empowerment; link persons withneeded services, benefits and entitlements, and other resources.
Supervision: Work isperformed under the supervision of a Program Administrator, Director orSupervisor. This position does not provide supervision to others.
? Adheres to agency designated model for serving the health andwellness needs of persons with or at risk of substance and/or co-occurringmental disorders; promotes consumer's self-determination, personalresponsibility and empowerment inherent in developing the personal knowledge,skills, and attitudes that promote health and wellness
? Engages individuals in health and wellness services throughmotivational interviewing and supportive counseling
? Screens individuals for risk of harm to self or others andprovides crisis intervention when necessary
? Links individuals to existing services, benefits counseling, andwellness activities to include, but not limited to, physical and behavioralhealth education, primary and specialty medical care, and resources to supportself-management of physical and behavioral health conditions
? Works as part of an interdisciplinary team to develop andimplement service or treatment plans, including taking the lead to re-engageindividuals who drop out of services
? Provides education, case management, and supportive counselingservices within areas of training, certification, education or other backgroundin the service area; seeks direction from Program Director/Administrator whenclient needs fall outside those areas
? Reviews and documents client response to services and adjustsinterventions, level of care, and/or array of services to better meet goals,based on consultation with the individual, significant others and/or additionalservice providers
? Modifies education and counseling to improve individual engagementand understanding, taking into consideration such characteristics as age,gender, primary language, disability, developmental level, sexual orientationor identity, ethnicity, and health status
? Monitors results of health-related labs and testing and sharesinformation with individuals and the treatment team, as applicable
? Knowledge of community resources
? Develops and/or maintains educational and reference materials onapplicable community resources to provide to individuals receiving services
? Knowledge of and compliance with specific requirements based onlaws, rules, contract, and/or accreditation standards
? Maintains productivity standards for assigned programs
? Performs related work as required and/or assigned
? Must possess valid Florida driver's license, industry accepteddriving history and maintain vehicle insurance required by State of Florida
Program-Specific Characteristics of Position:
The Children's HIV program utilizes Health Coaches to conductscreening and assessment of youths who agree to participate, specificallyfocused on risks for HIV and other sexually transmitted diseases (STDs). TheHealth Coaches in this program provide health education specific to riskfactors for HIV and other STDs, onsite testing, and counseling and education tohelp youths reduce risk.
? Training in Florida Department of Health HIV/AIDS 500/501
Program-Specific Scope of Responsibilities:
? Conduct screening of those youths who consent to services toidentify personal risk factors related to HIV, Hepatitis, and other sexuallytransmitted diseases, using approved screening and assessment instruments
? Provide individualized education on risk reduction, identifyingand emphasizing those personal and specific factors relevant to that youth
? Introduce youths to the ACTS educational video on HIV and sexuallytransmitted diseases and provide information on how to access and view thevideo
? Identify youths who do not have primary care services andfacilitate linkage to Tampa Family Health Centers, as well as referrals forinsurance and health coverage benefits
? Conduct individual pre-test counseling for HIV, following theprotocols of the Florida Department of Health 500/501 training
? With appropriate written consent, conduct rapid HIV testing onsiteat the JAC and provide post-test counseling, following the protocols of theFlorida Department of Health 500/501 training
? Provide follow up support, linkage, and counseling to youths whotest positive for HIV and other STDs, including linkage with the HillsboroughCounty Health Department
? Participate in health fairs and community outreach events relatedto HIV testing and services
? Assist in data collection and reporting for research related tothis program, including follow-up contacts at prescribed intervals with youthswho have received services
? Knowledge of confidentiality and reporting laws on results oftests for HIV, Hepatitis, and other sexually transmitted disease, and theirrelationship to 42 CFR Part Two and HIPAA
