Jobing Description

Recovery Liaison Back End Day Shift: Thurs., Fri., Sat., every other Wed. (Full-Time)

Under general supervision, participates as a member of the Recovery Service Team to provide coordination of care and assists in the discharge planning with the person or the guardian, partners, other involved agencies and supports identified by the guest. Acts as an advocate for the needs and rights of every person we serve.

Essential Job Functions

  1. Provides a welcoming environment for new participants using the Recovery Coaching model.
  2. Provides basic recovery support services in partnership with service team and documents interactions appropriately.
  3. Provides Coordination of Care with the guests, clinical sites, other partnering agencies, guardians, and the provider contractor.
  4. Participates and assist in developing a discharge plan based on interventions and the presenting problem.
  5. Assures time lines are met for court-ordered treatment, and other timely processes.
  6. Assists guests with referrals to other community agencies as indicted upon completion of assessments and/or discharge plan.
  7. Facilitates authorization for inpatient services and ensures accurate enrollment for ongoing service recommendations/ referrals.

Education, Licenses and Experience

  1. Bachelors degree in a field related to behavioral health or;
  2. Bachelors degree in any field and at least one (1) year of full-time behavioral health work experience; or
  3. Associates degree and at least two (2) years of full-time behavioral health work experience; or
  4. Licensed as a practical nurse, according to A.R.S. Title 32, Chapter 15, with at least two years of full-time behavioral health work experience; or
  5. High school diploma or GED and eighteen (18) college credit hours of post-high school education in a field related to behavioral health completed no more than four (4) years before the date the individual begins providing behavioral health services and two (2) years of full-time behavioral health experience; or
  6. High school diploma or GED and at least four (4) years of full-time behavioral health work experience

Additional Requirements, Certifications and Qualifications

  1. Current Healthcare Provider CPR Certification
  2. Current First Aid Certification
  3. Required to pass pre-placement physical examination
  4. Proof of negative TB test within the last 6 months, and annually.
  5. Completion of Therapeutic Options as required.
  6. Proficient in Microsoft office products; ability to type 30-40 w.p.m
  7. Demonstrated ability to effectively communicate both verbally and in writing
  8. Must meet all of the requirements of the Driver’s Safety Policy.

Working Conditions

Works in behavioral health clinical areas. Some risk involved in the event of agitated guests mitigated by team training and verbal de-escalation. Must possess visual and audio skills to complete job duties. Schedule may include long hours and travel throughout geographic region.

The physical requirements described herein are representative of those which must be met by an employee to successfully perform the primary functions of this job. Continuous standing; walking; handling; finger dexterity, eye hand-foot coordination; use of corrected vision; depth perception; wide field of vision; color vision; use of auditory, olfactory and tactile senses. Frequent bending over; stooping, kneeling; crouching; pushing; pulling; sitting; reaching; climbing/balancing on a step stool to reach supplies/equipment in cabinets; climbing stairs; carrying up to 25 pounds. Occasional crawling; running; pushing up to 250 pounds; pulling up to 250 pounds; lifting up to 50 pounds.
